📋 Clinical Overview

A topical combination product primarily used as a keratolytic, antipruritic, and counterirritant. Menthol provides a cooling sensation and mild local anesthetic effect, while salicylic acid promotes desquamation of hyperkeratotic skin by softening keratinocytes and disrupting intercellular bonds. It is a mainstay in Indian dermatology for treating hyperkeratotic and pruritic skin conditions.

💊 Dosage & Administration

Adult: Apply a thin layer to the affected area 1-2 times daily, or as directed by a physician. For scalp: Apply to dry scalp, leave for 5-10 minutes, then wash off with shampoo.

Note: For external use only. Wash and dry the affected area thoroughly before application. Apply with fingertips or cotton swab. Avoid contact with eyes, mucous membranes, and genital areas. Do not apply under occlusive dressings unless advised by a doctor. Wash hands after application.

⚠️ Contraindications

  • Known hypersensitivity to salicylates, menthol, or any component of the formulation.
  • Children under 2 years of age (risk of systemic salicylate absorption).
  • Patients with known glucose-6-phosphate dehydrogenase (G6PD) deficiency (salicylic acid can rarely induce hemolysis).

🔬 Mechanism of Action

The combination works synergistically. Salicylic Acid acts as a keratolytic by dissolving the intercellular cement substance (desmosomes) between corneocytes in the stratum corneum, leading to desquamation of scaly skin. It also possesses mild anti-inflammatory and comedolytic properties. Menthol activates the cold-sensitive TRPM8 receptors in the skin, producing a cooling sensation that masks pruritus (itch) and pain by a counterirritant mechanism. It also has mild local anesthetic properties.

  • Excessive dryness or peeling at application site

🤰 Special Populations

Pregnancy: Category C (US FDA). Salicylic acid is absorbed and crosses the placenta. Avoid large-scale or long-term use, especially in the third trimester (risk of premature closure of ductus arteriosus and prolonged labor). Use only if clearly needed and under medical supervision.

Driving: No effect. The product is for topical use only.
