📋 Clinical Overview

A potent triple-action topical combination used for the treatment of severe, inflamed, and infected dermatological conditions, particularly those of fungal origin with secondary bacterial infection. Clotrimazole provides antifungal activity, Clobetasol is a super-potent corticosteroid for rapid anti-inflammatory and immunosuppressive effects, and Neomycin is an aminoglycoside antibiotic effective against common skin pathogens. This combination is a mainstay in Indian dermatology for managing complex, acute presentations.

💊 Dosage & Administration

Adult: Apply a thin film to the affected area 1 to 2 times daily. Twice-daily application is typical for the first 3-7 days for acute control, then reduce to once daily. Total duration should not exceed 2-4 weeks.

Note: 1. Wash and dry the affected area thoroughly. 2. Take a small amount (pea-sized for an area the size of the palm) and apply a thin, even layer. 3. Gently rub in until absorbed. 4. Wash hands after application unless hands are the treatment site. 5. Do not cover with occlusive dressings unless specifically directed by a physician. 6. Avoid contact with eyes, mouth, nose, and mucous membranes.

⚠️ Contraindications

  • Hypersensitivity to any component (Clotrimazole, Clobetasol, Neomycin, or other aminoglycosides)
  • Viral skin infections (e.g., herpes simplex, chickenpox, vaccinia)
  • Fungal infections not caused by susceptible organisms (e.g., deep mycoses)
  • Primary bacterial skin infections not covered by Neomycin (e.g., Pseudomonas)
  • Tuberculous or syphilitic skin lesions
  • Perioral dermatitis, acne rosacea

🔬 Mechanism of Action

This combination exerts a synergistic therapeutic effect by targeting three key pathological components: infection (fungal and bacterial) and inflammation. Clotrimazole inhibits ergosterol synthesis in fungal cell membranes, leading to increased permeability and cell death. Clobetasol binds to glucocorticoid receptors, inhibiting phospholipase A2, thereby reducing the production of pro-inflammatory mediators like prostaglandins and leukotrienes. Neomycin inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to bactericidal activity against susceptible organisms.

🤕 Side Effects

  • Local burning or stinging sensation (usually transient)
  • Itching (pruritus) at application site
  • Dryness, redness (erythema), or peeling of skin
  • Skin irritation

🤰 Special Populations

Pregnancy: Category C (US FDA). Topical corticosteroids, especially potent ones, can be absorbed in amounts sufficient to produce systemic effects. Animal studies show teratogenicity. Use only if the potential benefit justifies the potential risk to the fetus. Avoid large areas, prolonged use, or occlusive dressings. First-trimester use should be particularly avoided.

Driving: No effect on driving ability from topical application.

🔄 Drug Interactions

Other Topical CorticosteroidsAdditive risk of local and systemic side effects (atrophy, HPA suppression).Major
Other Aminoglycosides (e.g., Gentamicin, Amikacin) - SystemicIncreased risk of ototoxicity and nephrotoxicity.Major
