📋 Clinical Overview

Povidone Iodine (5% w/w) is a broad-spectrum, topical antiseptic and disinfectant complex of polyvinylpyrrolidone (povidone) and elemental iodine. It is a widely used, time-tested antiseptic in the Indian healthcare setting, effective against a wide range of Gram-positive and Gram-negative bacteria, fungi, viruses, protozoa, and bacterial spores. It acts by releasing free iodine, which iodinates microbial proteins and oxidizes key cellular structures, leading to cell death. Its povidone component acts as a solubilizing agent, providing a sustained-release reservoir of iodine, reducing irritation compared to tincture of iodine.

💊 Dosage & Administration

Adult: Apply topically to affected area 1-3 times daily. For pre-op skin prep: apply generously, allow to dry. For surgical scrub: wash for 5 minutes. For wound irrigation: use diluted solution (0.1%-1%) and rinse thoroughly.

Note: 1. Clean the wound/area with sterile water/saline to remove organic debris. 2. Apply solution/ointment directly or with sterile gauze. 3. For pre-surgical prep, apply in concentric circles from the incision site outward. 4. Allow it to dry completely (for solutions). 5. Do not cover with occlusive dressings immediately unless specified. 6. For mucosal use (e.g., gargle), must be diluted as per instructions (typically 10-20 drops in a glass of water).

⚠️ Contraindications

  • Known hypersensitivity to iodine or any component of the formulation
  • Patients with pre-existing thyroid disorders (e.g., nodular goitre, autoimmune thyroiditis) where iodine load is undesirable, especially for large wound/lavage use
  • Neonates (especially preterm) due to risk of hypothyroidism and chemical burns - use with extreme caution and not for routine skin antisepsis

🔬 Mechanism of Action

Povidone Iodine is an iodophor that acts as a reservoir, slowly releasing free iodine (I2) in aqueous solution. The free iodine penetrates the cell wall of microorganisms and attacks key groups in proteins (particularly sulfhydryl -SH groups), nucleotides, and fatty acids. This leads to denaturation and coagulation of proteins, disruption of microbial electron transport, and oxidation of cellular structures, resulting in rapid cell death.

🤰 Special Populations

Pregnancy: Category C (US FDA). Iodine crosses the placenta. Prolonged or extensive use, especially on large wounds or mucous membranes, can cause fetal hypothyroidism and goiter. Use only if clearly needed, for short duration, and on limited areas. Avoid vaginal application during delivery.
