📋 Clinical Overview

A fixed-dose combination supplement designed to address metabolic, neurological, and vascular health, particularly in the context of diabetic complications, peripheral neuropathy, and mitochondrial dysfunction. Benfotiamine is a lipid-soluble thiamine (B1) analog with superior bioavailability. Levo-carnitine facilitates fatty acid transport into mitochondria for energy production. Resveratrol is a polyphenol with antioxidant and anti-inflammatory properties. Pyridoxine (B6) is a cofactor in numerous enzymatic reactions, including neurotransmitter synthesis and homocysteine metabolism.

💊 Dosage & Administration

Adult: One tablet/capsule once daily, or as directed by the physician. Often taken with a meal to enhance resveratrol absorption and reduce GI upset.

Note: Administer orally with a glass of water, preferably with a meal containing some fats (e.g., milk, nuts) to improve the absorption of fat-soluble benfotiamine and resveratrol. Do not crush or chew unless specified.

🔬 Mechanism of Action

The combination acts synergistically on multiple pathways. Benfotiamine increases intracellular thiamine diphosphate, a cofactor for transketolase, which redirects glycolytic intermediates (fructose-6-phosphate and glyceraldehyde-3-phosphate) away from the damaging triosephosphate-induced pathways (hexosamine, AGE, PKC, and NF-κB) implicated in diabetic complications. L-Carnitine transports long-chain fatty acids into the mitochondrial matrix for β-oxidation, improving cellular energy (ATP) production, especially in energy-demanding tissues like nerves and muscle. It may also improve insulin sensitivity. Resveratrol activates sirtuins (SIRT1), which promote mitochondrial biogenesis, reduce oxidative stress via Nrf2 pathway, and exert anti-inflammatory effects by inhibiting COX and NF-κB. Pyridoxine, as pyridoxal phosphate (PLP), is a cofactor for over 150 enzymes, including those involved in neurotransmitter synthesis (GABA, serotonin, dopamine), homocysteine metabolism (cystathionine β-synthase), and glycogen breakdown.

Pregnancy: Category C (US FDA not assigned for combo). Insufficient data. Benfotiamine and L-carnitine are likely safe, but resveratrol's phytoestrogen effect warrants caution. Use only if potential benefit justifies potential risk. Pyridoxine is commonly used for nausea in pregnancy.

Driving: No known effects. However, if dizziness occurs as a rare side effect, avoid driving.
