📋 Clinical Overview

A topical combination preparation containing a potent corticosteroid (Fluticasone Propionate) and a topical antibiotic (Mupirocin). It is primarily used for the treatment of inflammatory dermatoses with secondary bacterial infection, particularly those caused by Staphylococcus aureus and Streptococcus pyogenes. The combination provides anti-inflammatory, anti-pruritic, and antibacterial actions.

💊 Dosage & Administration

Adult: Apply a thin layer to the affected area 2-3 times daily. Rub gently. Duration: Usually 5-10 days, not to exceed 2 weeks.

Note: 1. Wash and dry hands and affected area. 2. Apply a pea-sized amount (or thin film) and rub in gently until it disappears. 3. Do not cover with occlusive dressings unless specifically advised by a doctor. 4. Avoid contact with eyes, nose, mouth, and mucous membranes. 5. Wash hands after application unless hands are the treated area.

⚠️ Contraindications

  • Hypersensitivity to fluticasone propionate, mupirocin, or any component of the formulation
  • Primary bacterial, viral (e.g., herpes simplex, chickenpox), or fungal skin infections without significant inflammation
  • Tuberculous or syphilitic skin lesions
  • Perioral dermatitis
  • Acne rosacea

🔬 Mechanism of Action

Fluticasone Propionate binds to cytoplasmic glucocorticoid receptors, leading to the synthesis of anti-inflammatory proteins (lipocortin) and inhibition of the production of pro-inflammatory mediators (prostaglandins, leukotrienes, cytokines). Mupirocin inhibits bacterial protein synthesis by reversibly and specifically binding to bacterial isoleucyl-tRNA synthetase, preventing incorporation of isoleucine into protein chains.

🤰 Special Populations

Pregnancy: Category C (US FDA). Animal studies show risk; no adequate human studies. Use only if potential benefit justifies potential fetal risk. Avoid large amounts or long-term use. Topical corticosteroids can be absorbed in amounts sufficient to cause systemic effects.

Driving: No effect on ability to drive or operate machinery.
