📋 Clinical Overview

Diphtheria, Tetanus, and Pertussis Toxoids (DTaP) is a sterile suspension of purified, inactivated toxoids from Corynebacterium diphtheriae, Clostridium tetani, and acellular pertussis antigens from Bordetella pertussis. It is a critical component of the National Immunization Schedule (NIS) in India, administered as a primary series and booster doses to induce active immunity against three life-threatening bacterial diseases. It is a non-infectious, subunit vaccine.

💊 Dosage & Administration

Adult: NOT USED in adults. Adults receive Tdap (reduced antigen content) or Td boosters.

Note: For intramuscular use only. Shake vial/ syringe well before use. The preferred site in infants and young children is the anterolateral aspect of the thigh. Do not inject intravenously, intradermally, or subcutaneously. Use a separate, sterile syringe and needle for each patient. Do not mix with other vaccines in the same syringe unless specifically licensed for such use.

⚠️ Contraindications

  • History of a severe allergic reaction (e.g., anaphylaxis) to a previous dose of DTaP or any component of the vaccine (including aluminum adjuvant, formaldehyde, or antibiotics like neomycin/polymyxin B used in some preparations).
  • Encephalopathy (e.g., coma, decreased level of consciousness, prolonged seizures) within 7 days of a prior pertussis-containing vaccine, not attributable to another identifiable cause.

🔬 Mechanism of Action

Contains purified, formaldehyde-inactivated exotoxins (diphtheria toxoid, tetanus toxoid) and purified, inactivated acellular pertussis antigens (such as Pertussis Toxoid (PT), Filamentous Hemagglutinin (FHA), Pertactin (PRN)). Upon intramuscular administration, these antigens are taken up by antigen-presenting cells (APCs), processed, and presented to helper T-cells (CD4+). This stimulates B-lymphocytes to proliferate and differentiate into antibody-producing plasma cells and memory B-cells. The aluminum salt adjuvant acts as a depot, prolonging antigen exposure and enhancing the immune response.

🤕 Side Effects

  • Local reactions: Pain, tenderness, erythema (redness), and swelling at the injection site (in ~20-40% of recipients).
  • Systemic: Fever (usually low-grade, <38.5°C), irritability, fussiness, drowsiness, anorexia (loss of appetite).
  • Crying: Prolonged crying (>3 hours) in a small percentage.

🤰 Special Populations

Pregnancy: This pediatric DTaP formulation is NOT recommended for pregnant women. Pregnant women should receive a single dose of Tdap (adult formulation) during each pregnancy, preferably between 27-36 weeks of gestation, to protect the newborn via passive antibody transfer.

Driving: No effect. However, fever or malaise post-vaccination may temporarily impair ability.
