📋 Clinical Overview

Hydroxypropylmethylcellulose (HPMC) 2% is a sterile, non-preserved, topical ophthalmic solution used as an ocular lubricant and a surgical aid. It is a semisynthetic, inert, viscoelastic polymer derived from cellulose. In the Indian context, it is widely used as an artificial tear substitute for dry eye disease and as a viscoelastic agent during ophthalmic surgeries like cataract extraction and intraocular lens (IOL) implantation to maintain space, protect corneal endothelium, and manipulate tissues.

💊 Dosage & Administration

Adult: **Lubricant:** 1-2 drops in the affected eye(s) as needed, typically 3-4 times daily or as directed. **Surgical:** As required by the surgeon to fill the anterior chamber, coat instruments/IOLs. Typically 0.2-0.8 mL per procedure.

Note: **Eye Drops:** Wash hands. Tilt head back, pull lower eyelid down, instill drop without touching tip to eye. Close eye gently for 1-2 minutes. **Surgical:** Used under aseptic conditions via a sterile cannula. Must be thoroughly aspirated/irrigated from the eye at the end of surgery to minimize post-op IOP spike.

🔬 Mechanism of Action

1. **As a Lubricant:** It acts as a demulcent, coating the ocular surface to protect and lubricate epithelial cells. Its mucoadhesive properties prolong residence time on the cornea, stabilizing the tear film and providing symptomatic relief from dryness. 2. **As a Surgical Viscoelastic:** It provides space maintenance in the anterior chamber, protects delicate tissues (corneal endothelium, iris) from mechanical trauma, facilitates manipulation of intraocular lenses and tissues, and acts as a tamponade to control minor bleeding.

  • Transient blurred vision immediately after instillation
  • Mild ocular discomfort or stickiness of eyelids
  • Temporary visual haze

🤰 Special Populations

Pregnancy: US FDA Category C (Animal reproduction studies not conducted). However, considered safe due to negligible systemic absorption. Can be used when clearly needed. Consult physician.

Driving: May cause transient blurred vision. Patients should wait until vision clears before driving or operating machinery.
