📋 Clinical Overview

Alpha glycerylphosphorylcholine (Alpha-GPC, choline alfoscerate) is a highly bioavailable choline-containing phospholipid precursor. It serves as a source of choline for the synthesis of the neurotransmitter acetylcholine and as a donor of phosphatidylcholine for neuronal membrane phospholipid synthesis. In the Indian context, it is primarily used as a nootropic and neuroprotective agent for cognitive disorders, including age-related cognitive decline and post-stroke recovery.

💊 Dosage & Administration

Adult: 400 mg to 1200 mg per day, administered in 1-2 divided doses. Common starting dose is 400 mg once daily, which can be increased to 400 mg twice daily based on tolerance and response.

Note: Administer orally with or without food. Capsules/tablets should be swallowed whole with a glass of water. For best absorption, it is often recommended to take it in the morning or split doses to avoid potential sleep disturbances.

⚠️ Contraindications

  • Known hypersensitivity to Alpha-GPC or any component of the formulation

🔬 Mechanism of Action

Alpha-GPC provides a highly bioavailable source of choline. It crosses the blood-brain barrier efficiently where it serves two primary functions: 1) It is hydrolyzed to release choline, which is a rate-limiting substrate for the synthesis of the neurotransmitter acetylcholine in cholinergic neurons. 2) It acts as a precursor for the synthesis of phosphatidylcholine, a major component of neuronal cell membranes, supporting membrane integrity and fluidity.

🤕 Side Effects

  • Headache (mild, transient)
  • Heartburn or gastrointestinal discomfort
  • Nausea
  • Insomnia (if taken late in the day)
  • Dizziness

🤰 Special Populations

Pregnancy: Category N (Not classified by US FDA). Animal reproduction studies are insufficient. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended in India for pregnant women.

Driving: May cause dizziness in some individuals. Patients should be cautioned about operating machinery or driving until they are certain the medication does not affect them adversely.

🔄 Drug Interactions

Scopolamine, Atropine (Anticholinergics)Alpha-GPC may antagonize the effects of anticholinergic drugs by increasing acetylcholine levels.Moderate
Cholinesterase Inhibitors (Donepezil, Rivastigmine, Galantamine)Additive cholinergic effects. May increase efficacy but also increase the risk of cholinergic side effects (nausea, diarrhea, bradycardia).Moderate
Muscarinic Agonists (e.g., Pilocarpine, Bethanechol)Additive cholinergic effects, potentially leading to excessive salivation, sweating, bradycardia.Moderate
Anticoagulants/Antiplatelets (Warfarin, Clopidogrel)Theoretical interaction due to phospholipid nature, but no clinically significant interactions reported. Monitor as a precaution.Minor
