Generic Pancreatin

Pancreatin (400mg)
Price: Low
Mfr: Various (e.g., Sun Pharma, Mylan) | Form: Enteric-coated tablet, Capsule

📋 Clinical Overview

Pancreatin is a digestive enzyme supplement derived from porcine pancreas. It contains a mixture of enzymes including amylase, lipase, and protease, which aid in the digestion of carbohydrates, fats, and proteins respectively. The 400mg strength typically refers to the total enzymatic activity or weight of the preparation.

💊 Dosage & Administration

Adult: Dosage is highly individualized based on fat intake and clinical response. Typical starting dose: 1-2 capsules (400mg each) with main meals and 1 capsule with snacks. Dosage may be titrated up to 50,000-75,000 units of lipase per kg per day in severe EPI, as per specific product labeling.

Note: Swallow capsules whole with a cold or room-temperature liquid during or immediately after meals. Do NOT crush or chew enteric-coated tablets/capsules. For patients unable to swallow capsules (e.g., children), the contents may be mixed with a small amount of soft, acidic food like applesauce or yogurt and swallowed immediately without chewing. Do not mix with hot food or alkaline substances.

⚠️ Contraindications

  • Known hypersensitivity to pork proteins or any component of the formulation.
  • Acute pancreatitis or acute exacerbations of chronic pancreatitis.
  • Severe, acute gastrointestinal obstruction.

🔬 Mechanism of Action

Pancreatin contains a standardized mixture of porcine-derived pancreatic enzymes: amylase, lipase, and protease. These enzymes are enteric-coated to protect them from gastric acid inactivation. Upon reaching the duodenum, the coating dissolves in the alkaline environment, releasing the enzymes. Amylase hydrolyzes starch into dextrins and sugars; lipase breaks down triglycerides into monoglycerides and free fatty acids; and protease cleaves proteins into peptides and amino acids. This enzymatic activity compensates for deficient endogenous pancreatic secretion, enabling proper digestion and absorption of macronutrients.

🤕 Side Effects

  • Abdominal discomfort or pain
  • Nausea
  • Flatulence
  • Diarrhea or constipation
  • Perianal irritation (with high doses)

🤰 Special Populations

Pregnancy: Category C: Use only if clearly needed. Animal reproduction studies have not been conducted. Consider risk vs. benefit. Essential for managing EPI in pregnant patients to ensure adequate nutrition.
