A fixed-dose combination therapy primarily indicated for the management of female infertility due to anovulation or oligo-ovulation. Clomiphene citrate is a selective estrogen receptor modulator (SERM) that induces ovulation by blocking estrogen receptors at the hypothalamus, leading to increased gonadotropin secretion. Coenzyme Q10 (Ubidecarenone) is a fat-soluble quinone and a vital component of the mitochondrial electron transport chain, acting as an antioxidant and cellular energy enhancer. In the context of infertility, it is believed to improve oocyte quality and mitochondrial function in ovarian cells, potentially enhancing the efficacy of ovulation induction and supporting endometrial receptivity.
Adult: One tablet (Clomiphene 25mg + CoQ10 50mg) orally once daily, usually starting on day 2, 3, 4, or 5 of the menstrual cycle for 5 consecutive days. The clomiphene dose may be titrated by the physician in subsequent cycles (e.g., to 50mg) based on response, while CoQ10 is typically continued at a fixed dose.
Note: Take at the same time each day, with or immediately after a meal containing fats to enhance the absorption of Coenzyme Q10. Swallow whole with a glass of water. The timing of the dose (morning/evening) is not critical but should be consistent.
Clomiphene competitively blocks estrogen receptors (ERs) at the level of the hypothalamus, inhibiting the normal negative feedback of circulating estrogens. This is misinterpreted as low estrogen levels, leading to increased pulsatile secretion of Gonadotropin-Releasing Hormone (GnRH) from the hypothalamus. This, in turn, stimulates the anterior pituitary to release Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH), promoting follicular growth and maturation, and ultimately triggering ovulation. Coenzyme Q10 serves as an essential cofactor in the mitochondrial electron transport chain (Complexes I, II, and III) for adenosine triphosphate (ATP) synthesis. It also functions as a potent lipophilic antioxidant, protecting cell membranes and LDL cholesterol from peroxidation. In ovarian physiology, it is postulated to improve mitochondrial bioenergetics in aging oocytes, reduce oxidative stress in the follicular fluid, and potentially improve endometrial lining quality.
Pregnancy: CATEGORY X. Clomiphene is contraindicated in pregnancy. This combination must be discontinued once pregnancy is confirmed. Coenzyme Q10 is generally considered safe in pregnancy, but the combination product should not be used.
Driving: May cause dizziness, visual disturbances (blurred vision, spots), or lightheadedness. Patients should be cautioned about driving or operating machinery until they are certain they are not affected.
