📋 Clinical Overview

A fixed-dose combination product containing a benzodiazepine (chlordiazepoxide) and an anticholinergic/antispasmodic agent (clidinium bromide). It is primarily used for the short-term management of anxiety disorders and associated somatic symptoms, particularly those involving the gastrointestinal tract, such as irritable bowel syndrome and peptic ulcer disease. The combination aims to address both the central anxiety and the peripheral autonomic hyperactivity.

💊 Dosage & Administration

Adult: One capsule/tablet three or four times daily, preferably before meals and at bedtime. The dosage should be individualized, starting at the lower end of the range. Usual range: 1-2 capsules 3-4 times/day. Maximum: 8 capsules/tablets (40mg chlordiazepoxide + 20mg clidinium) per day, only under close supervision.

Note: Administer orally with a full glass of water. Can be taken with or without food, but taking before meals may enhance effect on GI symptoms. Do not crush or chew sustained-release formulations. Avoid abrupt discontinuation after prolonged use.

⚠️ Contraindications

  • Known hypersensitivity to chlordiazepoxide, clidinium, other benzodiazepines, or any component.
  • Myasthenia gravis.
  • Narrow-angle glaucoma.
  • Prostatic hypertrophy with significant urinary retention.
  • Severe hepatic impairment.
  • Sleep apnea syndrome.
  • Acute pulmonary insufficiency.
  • Pregnancy (especially first trimester).

🔬 Mechanism of Action

The combination exerts a dual mechanism: Chlordiazepoxide produces anxiolytic, sedative, and muscle relaxant effects by potentiating GABAergic neurotransmission in the CNS. Clidinium bromide reduces gastrointestinal motility and secretory activity by competitively inhibiting muscarinic acetylcholine receptors in the smooth muscle and glands of the GI tract.

  • Dry mouth (xerostomia).
  • Blurred vision.
  • Constipation.
  • Fatigue, ataxia (unsteadiness).

🤰 Special Populations

Pregnancy: Category D (US FDA). Contraindicated, especially in first trimester. Benzodiazepines may cause fetal harm (floppy infant syndrome, withdrawal symptoms in neonate). Risk of cleft palate if used in first trimester. Should not be used in pregnancy.

Driving: Patients must be warned that this drug impairs alertness, coordination, and reaction time. It is dangerous to drive or operate machinery while taking this medication, especially at treatment initiation or after a dose change.
