📋 Clinical Overview

A fixed-dose combination topical otic/otic-sterile solution designed for the treatment of acute otitis externa and infected otitis media with perforation. It combines a broad-spectrum bacteriostatic antibiotic (Chloramphenicol), a potent corticosteroid (Prednisolone) for anti-inflammatory and anti-pruritic action, an acidifying/antimicrobial agent (Acetic acid), and a local anesthetic (Benzocaine) for symptomatic pain relief. This combination addresses infection, inflammation, and pain simultaneously, commonly used in Indian clinical practice for 'swimmer's ear' and other acute ear infections.

💊 Dosage & Administration

Adult: 2-4 drops, instilled into the affected ear canal 3-4 times daily. The patient should lie with the affected ear upward for 5 minutes after instillation.

Note: 1. Warm the bottle in hands for 1-2 minutes to avoid dizziness from cold drops. 2. Clean and dry the outer ear gently. 3. Lie on side with affected ear up. 4. Pull the outer ear backward and upward (adults) or backward and downward (children) to straighten canal. 5. Instill prescribed drops. 6. Stay in position for 5 minutes. 7. A sterile cotton plug may be placed loosely at the canal opening. DO NOT insert deep into canal. 8. Wash hands before and after. DO NOT let dropper tip touch ear, fingers, or any surface.

⚠️ Contraindications

  • Hypersensitivity to Chloramphenicol, Prednisolone, Benzocaine, Acetic acid, or any component
  • Viral infections of the ear canal (e.g., herpes simplex, varicella)
  • Fungal otitis externa (unless as part of a mixed infection where bacteria are primary)
  • Perforated tympanic membrane (as a relative contraindication for some clinicians, but this product is often used for infected perforations; caution is paramount)
  • History of Chloramphenicol-induced blood dyscrasias

🔬 Mechanism of Action

This combination exerts a multi-pronged therapeutic action. Chloramphenicol inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. Prednisolone induces phospholipase A2 inhibitory proteins (lipocortins), suppressing prostaglandin and leukotriene synthesis, thereby reducing inflammation, edema, and itching. Acetic acid creates an acidic environment (lowers pH) hostile to bacterial and fungal growth, particularly Pseudomonas aeruginosa and Candida. Benzocaine blocks sodium ion channels in neuronal membranes, preventing the initiation and conduction of nerve impulses, providing local analgesia.

  • Transient mild stinging or burning sensation upon instillation (due to Acetic acid)

  • Dryness or flaking of skin in the ear

🤰 Special Populations

Pregnancy: Category C (US FDA). Chloramphenicol crosses the placenta. 'Gray Baby Syndrome' is a risk in the third trimester if systemically absorbed. Prednisolone is also Category C. Use only if the potential benefit justifies the potential fetal risk. Topical application to a limited area (ear) is less risky than systemic administration, but should be avoided unless absolutely necessary.

Driving: Unlikely to affect driving ability. However, if dizziness or vertigo occurs as a side effect (ototoxicity), patients should not drive or operate machinery.
