📋 Clinical Overview

Benzydamine is a non-steroidal anti-inflammatory drug (NSAID) with potent local analgesic, anti-inflammatory, and antiseptic properties. In the 0.15% w/v concentration, it is primarily formulated as a topical solution for oropharyngeal use (mouthwash/gargle) and as a topical gel/spray. It is a first-line agent in India for managing pain and inflammation associated with various oral and throat conditions. Its unique mechanism provides rapid symptomatic relief without significant systemic absorption.

💊 Dosage & Administration

Adult: Mouthwash/Gargle: 15 mL (approximately one tablespoonful) of 0.15% solution, undiluted, to be used as a mouthwash/gargle for 30 seconds to 1 minute, then expelled. Frequency: Every 1.5 to 3 hours, or as directed by physician. Do not swallow. Spray: 4-8 sprays directed onto the affected area, 3-4 times daily.

Note: For mouthwash: Measure 15 mL, rinse/gargle thoroughly for at least 30 seconds ensuring contact with all affected areas, then spit out. Avoid eating or drinking for at least 15-30 minutes after use for optimal effect. Do not dilute the solution unless specified (some brands recommend dilution for children). For spray: Shake well before use. Direct nozzle towards affected area and press actuator.

⚠️ Contraindications

  • Known hypersensitivity to Benzydamine HCl or any component of the formulation.
  • Patients with a history of asthma, urticaria, or allergic-type reactions precipitated by aspirin or other NSAIDs.

🔬 Mechanism of Action

Benzydamine exerts its effects through multiple mechanisms: 1) Inhibition of prostaglandin synthesis by blocking cyclooxygenase (COX) enzymes, reducing inflammation and pain. 2) Stabilization of cell membranes, inhibiting the release of inflammatory mediators like histamine, bradykinin, and cytokines. 3) Local anesthetic-like action by interfering with neuronal signal transduction. 4) Antiseptic properties against a broad spectrum of gram-positive and gram-negative bacteria, as well as Candida albicans.

  • Transient local numbness or stinging/burning sensation at application site.
  • Altered taste perception (bitter taste).
  • Dry mouth or feeling of thirst.
  • Mild local irritation.

🤰 Special Populations

Pregnancy: Category C (US FDA). No adequate and well-controlled studies in pregnant women. Use only if clearly needed and potential benefit justifies potential risk to the fetus. Systemic absorption is negligible, limiting risk.

Driving: Unlikely to affect ability to drive or use machines, as systemic effects are negligible.
