📋 Clinical Overview

A fixed-dose combination therapy primarily indicated for the management of anovulatory infertility in women. Clomiphene citrate is a selective estrogen receptor modulator (SERM) that induces ovulation by blocking estrogen receptors at the hypothalamus, increasing gonadotropin secretion. Coenzyme Q10 (Ubiquinone) is a fat-soluble quinone and a vital component of the mitochondrial electron transport chain, acting as an antioxidant and supporting cellular energy (ATP) production. In this combination, CoQ10 is theorized to improve oocyte (egg) quality and mitochondrial function in ovarian cells, potentially enhancing the efficacy of ovulation induction.

💊 Dosage & Administration

Adult: One tablet (Clomiphene 50mg + CoQ10 50mg) orally once daily, usually for 5 days, starting on day 2, 3, 4, or 5 of the menstrual cycle (spontaneous or induced). Dosage of clomiphene may be increased to 100mg/day in subsequent cycles if ovulation does not occur. CoQ10 dosage is fixed at 50mg in this combination. The total duration of therapy should not exceed 6 cycles.

Note: Take with a full glass of water. For optimal absorption of CoQ10, it is recommended to take the tablet WITH A MEAL, preferably one containing fats/oils (e.g., milk, curd, nuts). Take at approximately the same time each day during the treatment cycle.

⚠️ Contraindications

  • Pregnancy (Clomiphene is contraindicated; Category X).
  • Liver disease or history of cholestatic jaundice with prior clomiphene use.
  • Uncontrolled thyroid or adrenal dysfunction.
  • Organic intracranial lesions (e.g., pituitary tumor).
  • Ovarian cysts (not due to PCOS) or enlargement.
  • Abnormal uterine bleeding of undetermined origin.
  • Known hypersensitivity to clomiphene citrate, coenzyme Q10, or any excipient.

🔬 Mechanism of Action

Clomiphene competitively blocks estrogen receptors (ERs) at the level of the hypothalamus, inhibiting the normal negative feedback of endogenous estrogen. This is misinterpreted as low estrogen levels, leading to increased pulsatile secretion of Gonadotropin-Releasing Hormone (GnRH) from the hypothalamus. This, in turn, stimulates the anterior pituitary to secrete increased amounts of Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH). The rise in FSH promotes follicular recruitment, growth, and maturation in the ovary, culminating in ovulation. Coenzyme Q10 is an essential cofactor in the mitochondrial electron transport chain (Complexes I, II, and III), crucial for aerobic cellular respiration and adenosine triphosphate (ATP) generation. In ovarian physiology, it acts as a potent antioxidant, mitigating oxidative stress in follicular fluid and the oocyte, and provides energy for the high metabolic demands of developing oocytes and supporting granulosa cells.

  • Vasomotor flushes (hot flashes) - very common (~10%).
  • Abdominal discomfort/bloating.
  • Breast tenderness.
  • Nausea, headache.
  • Abnormal uterine bleeding/spotting.
  • Ovarian enlargement/cyst formation.

🤰 Special Populations

Pregnancy: CONTRANDICATED (Pregnancy Category X). Clomiphene is teratogenic in animal studies. If pregnancy occurs during therapy, discontinue immediately. There is no evidence of harm from CoQ10 in pregnancy, but the combination must be stopped. Patients should be advised to stop medication if they suspect pregnancy.

Driving: Caution advised. Clomiphene may cause visual disturbances (blurring, spots), dizziness, or lightheadedness, which may impair the ability to drive or operate machinery.
