📋 Clinical Overview

Benzoyl Peroxide 4% w/w is a topical antimicrobial and keratolytic agent, considered a first-line treatment for mild to moderate inflammatory acne vulgaris in the Indian clinical setting. It is available over-the-counter (OTC) in various formulations like gels, creams, and washes. It exerts its therapeutic effect through potent antibacterial action against Cutibacterium acnes (formerly Propionibacterium acnes) and comedolytic activity.

💊 Dosage & Administration

Adult: Apply a thin layer to affected areas once daily, preferably in the evening. Frequency may be increased to twice daily (morning and evening) as tolerated.

Note: 1. Wash and gently pat dry the affected area. 2. Apply a pea-sized amount for the entire face or a thin layer to other areas. 3. Rub in gently and thoroughly. 4. Wash hands after application. 5. May be used under non-comedogenic moisturizer to reduce dryness. Avoid contact with hair and colored fabrics.

⚠️ Contraindications

  • Known hypersensitivity to Benzoyl Peroxide or any component of the formulation
  • Contact with eyes, mucous membranes, and broken or highly inflamed skin

🔬 Mechanism of Action

Benzoyl Peroxide exerts a triple action: 1) Antibacterial: It releases free oxygen radicals which oxidize bacterial proteins in Cutibacterium acnes, leading to bactericidal effect. 2) Keratolytic/Comedolytic: It promotes desquamation of the stratum corneum and helps expel existing comedones. 3) Anti-inflammatory: It may reduce the chemotactic factors and reactive oxygen species in the follicle.

🤰 Special Populations

Pregnancy: US FDA Pregnancy Category C. Animal studies are inconclusive. No adequate, well-controlled studies in pregnant women. Use only if clearly needed and potential benefit justifies potential risk to the fetus. Minimal systemic absorption reduces risk.

Driving: No effect on ability to drive or operate machinery.
