📋 Clinical Overview

A fixed-dose combination topical preparation containing a potent corticosteroid (Betamethasone) and an aminoglycoside antibiotic (Neomycin). It is primarily used for the treatment of inflammatory dermatoses with secondary or suspected bacterial infections. The combination provides anti-inflammatory, immunosuppressive, antipruritic, and antibacterial effects. In the Indian context, it is widely prescribed for various skin conditions but requires judicious use due to the risk of antibiotic resistance and steroid-induced skin atrophy.

💊 Dosage & Administration

Adult: Apply a thin film to the affected area 1 to 2 times daily. Frequency depends on severity; often initiated as twice daily and reduced to once daily as condition improves.

Note: 1. Wash and dry the affected area gently. 2. Apply a thin layer and rub in lightly until it disappears. 3. Do not cover with occlusive dressings unless specifically advised by a doctor. 4. Wash hands before and after application, unless hands are the treatment area. 5. Do not use on the face, groin, or axillae for prolonged periods. 6. Complete the prescribed course; do not stop abruptly if used for more than 2 weeks.

⚠️ Contraindications

  • Hypersensitivity to betamethasone, neomycin, other aminoglycosides, or any component of the formulation
  • Viral skin infections (e.g., herpes simplex, chickenpox, vaccinia)
  • Fungal skin infections (e.g., candidiasis, tinea)
  • Tuberculous or syphilitic lesions of the skin
  • Perioral dermatitis
  • Acne vulgaris
  • Application to the eyes or conjunctiva

🔬 Mechanism of Action

Betamethasone exerts potent anti-inflammatory, immunosuppressive, vasoconstrictive, and antipruritic effects by inducing phospholipase A2 inhibitory proteins (lipocortins), which inhibit the release of arachidonic acid and subsequent synthesis of prostaglandins and leukotrienes. It also inhibits the release of pro-inflammatory cytokines (IL-1, TNF-α) and reduces inflammatory cell migration. Neomycin is a bactericidal antibiotic that binds to the 30S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis and leading to cell death.

🤰 Special Populations

Pregnancy: Category C (US FDA). Topical corticosteroids, including betamethasone, can be absorbed in amounts sufficient to cause systemic effects. Animal studies show teratogenicity. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid large doses or prolonged use.

Driving: No effect on driving ability when used topically as directed.
