📋 Clinical Overview

Nicotinamide (4%) is a topical formulation of the water-soluble amide form of Vitamin B3 (Niacin). It is a well-established therapeutic agent in dermatology, primarily used for its anti-inflammatory, sebum-regulating, and skin barrier-enhancing properties. It functions as a precursor to essential coenzymes NAD+ and NADP+, playing a crucial role in cellular energy metabolism and repair. In the Indian context, it is a first-line and widely recommended treatment for mild to moderate inflammatory acne vulgaris and is also used for managing hyperpigmentation and rosacea.

💊 Dosage & Administration

Adult: Apply a thin layer to the affected area twice daily (morning and evening) after cleansing and drying the skin.

Note: 1. Cleanse skin with a mild cleanser and pat dry. 2. Take a pea-sized amount for the entire face. 3. Apply a thin, even film, gently massaging until absorbed. 4. Can be used under moisturizer and sunscreen. 5. For combination products (e.g., with clindamycin), follow specific brand instructions.

⚠️ Contraindications

  • Known hypersensitivity to Nicotinamide or any component of the formulation

🔬 Mechanism of Action

Nicotinamide exerts its therapeutic effects through multiple pathways: 1) Anti-inflammatory: Inhibits the production of pro-inflammatory cytokines (IL-1, IL-6, IL-8, TNF-α) and the enzyme phosphodiesterase, reducing neutrophil chemotaxis and reactive oxygen species. 2) Sebum Regulation: Reduces sebum production by inhibiting sebocyte lipid synthesis. 3) Skin Barrier: Increases biosynthesis of ceramides, free fatty acids, and cholesterol in the stratum corneum, improving barrier function and reducing transepidermal water loss (TEWL). 4) Pigmentation: Inhibits the transfer of melanosomes from melanocytes to keratinocytes.

🤕 Side Effects

  • Mild, transient burning sensation
  • Itching (pruritus)
  • Dryness or peeling of skin (xerosis)
  • Erythema (redness)
