A fixed-dose combination topical preparation designed for the treatment of mixed or complicated otitis externa and certain dermatological infections. It combines a broad-spectrum fluoroquinolone antibiotic (Ofloxacin), a potent topical corticosteroid (Beclometasone dipropionate), and a broad-spectrum imidazole antifungal (Clotrimazole). This combination provides comprehensive coverage against common bacterial and fungal pathogens while simultaneously reducing inflammation, pruritus, and edema, making it a first-line choice for otomycosis and infected eczematoid dermatitis in the Indian clinical setting.
Adult: For Otitis Externa: Instill 2-3 drops into the affected ear(s) twice daily (morning and evening) for 7-14 days, or as directed by the physician. For Skin: Apply a thin film to the affected area twice daily.
Note: 1. For Ear: Warm the bottle in hands for 1-2 minutes. Lie on side with affected ear upward. Pull the outer ear backward and upward (adults) to straighten canal. Instill drops. Remain in position for 5 minutes. Repeat for other ear if needed. 2. For Skin: Clean and dry the area thoroughly. Apply a thin layer and rub gently. Wash hands before and after application unless hands are the treated area. Do not cover with airtight dressings (occlusion) unless specifically advised. Do not use in or near the eyes.
This combination exerts a triple action: 1) Bactericidal action against susceptible gram-positive and gram-negative bacteria (Ofloxacin). 2) Anti-inflammatory, antipruritic, and vasoconstrictive action (Beclometasone). 3) Fungistatic/Fungicidal action against dermatophytes, yeasts (Candida), and Malassezia species (Clotrimazole).
Pregnancy: Pregnancy Category C (US FDA). Data on topical use in pregnancy is limited. Beclometasone and Clotrimazole are considered probably safe in limited amounts, but Ofloxacin is generally avoided in pregnancy due to arthropathy risk in animal studies. Use only if the potential benefit justifies the potential risk to the fetus. Use the minimum effective dose and area.
Driving: Unlikely to affect driving ability. However, if dizziness or tinnitus occurs as a side effect (otic use), patients should be cautioned about operating machinery or driving.
